After giving birth, families are introduced to the Newborn Navigator who will explain the service and inquire about scheduling the visit. Visits are currently occurring remotely. Prior to the visit, participants will receive a Welcome Baby Bag with newborn essentials. During the visit, the Newborn Navigator will support the family by understanding how their transition from the hospital home has been, inquire about existing needs, and provide access to community resources and referrals.<\/p>\n\n\n\n<p>Should the Newborn Navigator determine a family is eligible and could benefit from Enhanced Navigation, an additional five home visits will be provided.
