Navigate the Manchester Stars

Ready Set LAUNCH at Amoskeag Health

Ready Set LAUNCH is a FREE, one-time newborn home visit service available to all Manchester residents who give birth at the Elliot Hospital. After giving birth, families are introduced to the Newborn Navigator who will explain the service and inquire about scheduling the visit. Visits are currently occurring remotely. Prior to the visit, participants will receive a Welcome Baby Bag with newborn essentials. During the visit, the Newborn Navigator will support the family by understanding how their transition from the hospital home has been, inquire about existing needs, and provide access to community resources and referrals.

Should the Newborn Navigator determine a family is eligible and could benefit from Enhanced Navigation, an additional five home visits will be provided. These additional visits will provide additional resources and targeted support to address specific needs

Home visits offered virtually and when safe they will occur at participants’ homes or in the community at a mutually agreed upon location.

Eligibility Requirements

Ready Set LAUNCH is currently available to all Manchester residents giving birth at the Elliot Hospital. There are no other eligibility requirements. Newborn Navigator and hospital staff identify qualifying clients based on residency - no referral needed.
